Looks like a bad case of ich. Raise the temp and medicate.
See here for more ideas.

I dont have a nitrate tester yet - PH is around 7.4 , ammonia is non-existant...... Temp is around 78 usually.... Tank has been up for about 2 months now. The first fish to die was a Gold Seam/Nugget - had him for about a month. Then the next 2 were little l-135s i bought along with the l-02 about 2 weeks ago. The calvus thats in there still looks to be perfectly healthy. But the l-02 is still alive - but covered in ich.... :(

Kinda my first attempt at a cat/pleco tank - always was a cichlid guy before.

Check your nitrite levels. They are the second stage in the tank cycling, and the ones that most people lose fish to, IME. Definitely do 30% water changes and raise the temperature to 82 degrees to help combat the ich problem. Also stop feeding the tank for a few days, in case it IS a waste buildup issue.
