My Otocinclus Vittatus recently got a yellow head and part of his body also.
He/she looks fine, eats well, is active.
It looks almost like this:

I couldn't find a clear answer online so I post it here to you experts. Should I be worried? I also have a group of cocama's, maybe it could infect them?

A number of Loricariid species are documented to depigment like that. For some species, it's routine and cyclical.
I had a group of Hemiancistrus guahiborum that yellowed. They lived for several years with that color then died when their tank had a filtration failure.
As Jeanne said, a photo would be helpful.
Cheers, Eric

Thank you Jeanne and Eric.
I will try to catch him with my phone, but it looks just like in de link: a yellow head and part of his upper body also. Can it be old age? I had him/her for about 5 years now and I got it fully grown.

Looks okay to me. As long as it keeps eating and behaving like the others, I'd say you're fine.
Good luck.

Thanks Eric. Funny to see a yellow lightbulb swimming in the tank
